ATHLETIC PHYSICALS
WHEN: Thursday, Apr 28, 2022
After School.
Incoming 7th grade (current 6th grade) physicals will not begin until 4:30 or after they get back from their band competition.

COST: $10
WHO: Physicals are required for all incoming 7th, 9th, and 11th graders that plan to participate in athletics.
Also, any student that answered yes to question 1,2,3,4,5,or 6 on the medical history form must receive a new physical.
FYI:
Forms were sent home with students earlier this week. If a new form is needed, we do have some extras in the middle school office.
Getting your student’s physical today is not required. It is a convenient and inexpensive option. However, you may choose to take your to any doctor of your choice for their physical. Athletes will not be able to participate in any athletic activities for next school year without the required paperwork and physical.
