7th Grade Immunization Requirements

Dear Parent/Guardian:

Students entering 7th grade are required to have a vaccine dose of Tdap (tetanus, diphtheria, pertussis) and of MCV (meningococcal conjugate) for individuals who have not already received those vaccines. These two vaccines are commonly given on or around a person’s 11th birthday.

Please contact your health care provider or local health department if your child needs these vaccines. Please be prepared, on the day your 7th grader is scheduled to come pick up his/her schedule (Meet the Teacher Night) or by the first day of school, to provide proof of immunization (i.e., Certificate of Immunization [NC Immunization Report]) or the notarized affidavit exemption form.

If you would like to provide the proof of immunization to the school prior to those dates, you can email them to SHEA SCHAFER at sschafer@eastland.esc14.net or bring them to the front office during regular school hours beginning on Tuesday, the 22nd of August.

Schools are required by law to prohibit any student from attending school until proof of immunization of required vaccinations have been supplied to the school office. If the student does not have the required immunizations, they will have to be sent home from school.
